Patient Information
We will need to do a complete review of your dental and relevant medical history, via a few simple forms. If you like, you may speed things along by downloading the patient forms below.
Finances and Insurance
Our patient care manager will work with you to make the dental care you need affordable, whether or not you have dental insurance. You will find more detailed payment information on the Financial Policy page.
X-Rays
If you have had dental X-rays in the last six months, please bring them to your first appointment. If not, we will conduct a complete X-ray of your teeth and mouth.
Oral Examination and Consultation
We will need to conduct a comprehensive exam and consultation to discuss any existing problem or concerns.
